Sep 15th 2016!⃝In my opinion? Heroin, 100%--both from the addict's point of view, alongside friends and family.
Most of the lyrics have been covered when concerning the song being bled out to reflect using heroin, trying to come off it, or being at the opposite end of watching someone wither away under its devastating grip.
"Heave the needle, hollow needle, piercing through another victim." Doesn't need much more explanation than that.
"Crouched over, you were not there, living in fear." He's seen someone overdose. I think this mingles with the lyrics, "Should I, could I?" for the simple fact that it speaks through both the addict and friends/family.
"SHOULD I even try to stop them anymore?"
Frustration, fear of losing someone close to you, and unbiased worry as to how much more of someone's inner demons you can take.
"COULD I really, honestly quit?
This disease often tells us we can't.

"This time you've tried all that you can, it's turning you red." Again, frustration.
Know this: until the addict's ready to stop, their is nothing ANYONE can say or do for them.
"Die, withdrawal."
There aren't any old heroin addicts out there. To me, these are the two end results if you were to ever play in heroin's web. Death, or cleaning up.
"Hiding cold sweats, quivering lips." Self explanatory: Signs of withdraw.
"Heave the needle, hollow needle, piercing through another victim." Again, self explanatory.
One I didn't see covered, however, that sold me on the whole heroin theory:
"Limbs tight, skin tight" is a heroin addict tying off before we shoot up.
