Jun 10th 2018!⃝the rest of these are really literal, and kind of only looking at the lyrics at face value. I think there is a lot of metaphorical stuff going on. To me this is an internal conflict. A struggle to move on, and a battle with one's old self, unable to let go. The old self being represented by the patient in the song, and the new self by the narrator.
